The department where I did my Ph. D. required that I submit a do***ent 
for credit outlining what I intended to do for my research.  Since I had 
a rough idea for my original topic, there was little discussion involved.

I had to change my topic after spending a year on it after the 
department members of my committee at the time shot it down.  I 
submitted a revised do***ent to my supervisor, though not as formal as 
the first one, as I recall.
