Questions on the number of MDs who graduate every year

Q1:  How many MDs graduate every year from American colleges/
universities?
Q2:  I heard many years ago that this number is around 30,000, and
that this number is set by the AMA.  Is this true?
Q3:  How has this number changed over the years/decades?
