If the deceased used the mother's name as his surname, it seems likely
that
O.W. means Out of Wedlock. If not, maybe someone else will come up with a
guess.
As for "dis'd" maybe it means deceased and the person who filled out the
form was misspelling: diceased. That seems like a stretch. Do you know
whether the mother was deceased when her son died? If not, my guess won't
work.

Sorry for the brevity. This is for a male relative who died in the
Bronx, New York in 1936. Under Father's Name is (dis'd) O.W. and under
Mother's Name is Emma Stiebeling, (dis'd).
